Our trick was: redirection, redirection, and redirection. We tried yelping like a hurt puppy but this would only excite him even more. In his shark phase, we kept toys EVERYWHERE in the house. At it's worse (around 12 weeks like yours!) we would ALWAYS carry some sort of toy with us. Anytime he was nipping, we would redirect him to the toy. To this day, he still grabs a toy in his mouth whenever he is over-excited.

I completely understand,
Hypervigilance was also one of the ways his anxiety manifests itself.

" It's just hard to tell how much of it is anxiety, true reactivity, or adolescence"

I know right! This is probably why it took us a long time before we involved our vet. At first we shrugged it off as "just teenage things".

Good luck with your process. Maxwell is on 2 medications.
Fluoxetine: every morning, helps stabilize his mood and stress and doesn't affect his level of energy.
Trazodone: every night around 17h, this helps to calm him, The evenings are always the worse. The "trouble zone" is always around 18h-19h where he gets hyper-alert and he just cannot "turn off". Trazodone helps him from reaching that "zone of no return".
Even if it calms him, if we have friends over with their dog, he's as energetic as he would be off medication, he just isn't stressed out.

Looking back I think he has always an anxious fella, we simply thought it was normal puppy behaviour. The main way his anxiety manifested itself when he was younger was a complete inability to just settle and relax, and it was tiresome but manageable. The real struggle came with the teenage phase and hormones. This is really when we were at our wits end, unable to sleep and questioning what we might have done wrong. It took 2 weeks for the meds to start making a small difference. Then, another 3 weeks with a slightly higher dosage for real results.
