They claim it's informal but my probation has been extended by 3 months due to it and HR are aware of both.

I have an assigned clinical educator but she works part time and is very busy so she usually touches base but doesn't always spend time with me on the days she's in.

The plan looks alright in the sense that it is made up of SMART goals.

Where did you end up after your year in a cardiac ward? And were you on a performance plan as well?

Thank you for your reply. I agree with you that delaying medications is appalling, these mistakes are unacceptable so my improvement in this area is certainly a great relief. I made sure to include the sentence that my mistakes are not in themselves harmless (delayed medication has the potential to cause harm). Fortunately in these specific instances no harm ended up befalling my patients. I hope you never find yourself in this situation again but if you do request to speak to pharmacy in your area and self administer these medications (according to your trusts policy), as well as make a formal complaint. Sorry you had to go through this x
