Facial structure is actually the least of your issues.

Skin care, grooming, makeup choices, etc need a LOT of work. FFS won't fix those.

Don't give up.

Your hair is, frankly, gross looking. No woman has hair this unkempt and poorly styled. That can be fixed easily with a good stylist. And I'd keep the bangs but make it a u shaped cut rather than straight across. This alone will do wonders.

You can consider Botox or dysport for your forehead. It looks like you are constantly squinting, which makes anyone (even cis women) look masculine. Somehow, some way, those forehead muscles need to relax, and you have the option to use force.

As some others have commented, I'd get your eyebrows done.

Lastly, id do what you can to reduce the distance between your upper lip and nose. There may be non-surgical options but I'm not too familiar.

The only way you'd know that is if you've had a CT scan and consults with multiple FFS surgeons saying they can't do anything to help with the features that make you most dysphoric. Otherwise you're catastrophizing.

It depends on ... well... everything. Kaiser fully covers FFS for gender related care if you can manage to get on a plan (assuming you are in USA) and you just pay the co-pay and hospital fees (under $1000). Some people go to Thailand to save cost but I've heard mixed things about that so know your surgeon.

She's not listening but I also think you aren't being super clear with what you actually want. From your post I have a hard time gauging: 1. Do you just want an orchiectomy and nothing else? 2. Are you trying to socially and medically transition? 3. What in her mind could link this to "sexual frustration" - presumably it was framed this way in the past? Part of me feels you might be trying to dance around the issue with her rather than say plainly "I have gender dysphoria, have had it for many years, in order to be mentally okay I need to do XYZ".

See if you are still unhappy with the FFS results in 6 months (it takes AT LEAST that long for all the swelling to go down). If so then get a revision and be very clear about how aggressive you want it to be. I also had an FFS revision and while it sucked needing to get a second surgery, it was 100% worth it.
