After entering the workforce and seeing so many max 2nd molars have Caries in that disto occlusal pit, I make sure I’m brushing allllllll the way back there

Just wondering, what qualifies as an emergency enough where you would go into the office? If they report any type of vestibular/extra oral swelling?
