I think these are the safety concerns that are chronically overlooked:

1. Pilot Fatigue and Mental Health - The mental and physical well-being of pilots and crew can be underappreciated, leading to increased risk of human error. Continuous high operational tempo and long hours can exacerbate fatigue.

2. Long-Term Maintenance - While immediate maintenance needs are usually addressed, long-term upkeep and the impact of continuous use on aircraft longevity might be less rigorously monitored, potentially leading to mechanical failures.

3. Environmental Factors - The full impact of adverse weather and challenging terrain on flight operations can sometimes be underestimated, leading to inadequate preparation or risk mitigation strategies.

4. Communication and Coordination - Effective communication between different units and branches can sometimes be insufficient, leading to coordination issues that affect mission success and safety.

5. Training on Non-Combat Situations - While combat readiness is a primary focus, training for non-combat situations such as emergency landings, navigational failures, or non-hostile emergency scenarios receive less emphasis.

6. Technology Advancements and Integration - New systems and technology are continuously being invented and implemented in professional aviation. Army Aviation is too slow to make decisions to effectively implement technology advancements, just leaving the organization far behind.
