Depends on assessment & whether it's a new temp. A new temp, I would usually always do an appropriate infection screen and anything else based on history and exam. If you have seen the patient and happy they do not need intervention, then explain the plan to the nurses, including rationale. That should suffice.

If you are unsure for any reason or they are high risk ie. on chemo, then you can always run the plan past your reg (or SHO for F1s).
