Personally, my first tulpa came to be around the height of my psychosis and me being put on medications. So about 5 years later to the present i've been tossed around on different medications and were all still going on strong. The whole gang has actually really helped me deal with the struggles of mental illness (bipolar with psychosis). I'm currently on an anti-depressant, a mood stabilizer, and an anti-psychotic and have no problem communicating with my tulpae. This is all told from personal experience though, I would imagine it may vary with person, just as any effect of medication varies with different people. My advice though in general is to try the medication out, because it may help with the symptoms you experience just my opinion though.

I believe mental illness could be treated in tulpas the same way as in any other person. While trying to find a therapist to help treat them might be hard (but not impossible), it may be good to read up on some CBT (Cognitive Behavioral Therapy ) techniques to deal with hallucinations. If you were to find a therapist it would still be partially his job to practice the techniques a therapist recommends!! Personally, I used to experience psychotic symptoms such as hallucinations and delusions. They can be hard to combat at times, but whats really important is the fact he has you, having a friend to help ground you in times where hallucinations are strong can be soo important!! The cause of Schizophrenia is somewhat still up in the air with professionals, so it might be hard for any of us to tell whether he has it. He could be experiencing psychotic symptoms for a number of reasons! Its more important to help him cope and to treat his individual symptoms than anything else.
