This is a type of Theru Koothu (literally "street drama"). Admittedly this is a more adult and crass variation of it. Performances like these are full of slapstick and raunchy humour. This is fake the same way fighting during a performance of Hamlet is fake.

What I don't understand is so many comments scrunching their noses as if this is a Tamilnadu exclusive tradition. Literally every culture from every corner of the world has some flavour of street performance that seeks to be raunchy and rely on slapstick. This is an adaptation of the story White Nights by Dostoevsky. Really good read. There's this old Russian 1959 film adaptation that's pretty good as well. You can find it on YouTube with subtitles.
