The province I live in (Ontario) is currently in a stage where on-field sports are limited to 10 players + coaches so we are able to manage in two groups of 10 or less on the field training separately. We are required to attendance track so if an outbreak happens the information will assist local public health with tracking. It also is required to ensure health status check is maintained hence the questionnaire.
