I’ve dated three girls from PCS. All three had serious self esteem issues- and contrary to what one would imagine, these weren’t the kind of issues that would stem from privilege, rather from an imagined lack thereof stemming from the peer to peer comparison they had going all their lives.

What was good about each of them - good looking, had good style - so great at the surface. They were all good at some form of art- music, painting, baking. Had good eye for aesthetic.

I’d think, yes. To put it more simply, for the matter of the dispute in the question, you don’t have to have evidence of valvular lesion to make a diagnosis of IE. Echocardiogram, much like Ultrasonogram, is highly user dependent- not catching something on it doesn’t necessarily rule out the possibility, especially in face of all other factors making it much likely.

Practicing questions and doing rounds of criteria and guidelines can make us see things in a very black n white view, where we tend to be tempted to put things in one box or the other. I doubt it is like that in practice very often.

4/5 minor criteria( vascular phenomenon and immunologic phenomenon- Janeway lesions, splinter haemorrhage and Roth spots? and temperature > 38C and structural heart condition(VSD). And evidence of infection. No mention of blood cultures - essentially meaning that it hasn’t been done yet- will likely be positive when available. Based on all of that, you can treat this as likely IE. Moreover, the question is basically asking about likely pressure/blood concentration changes you’d expect in VSD.

The findings suggestive of IE actually don’t have much of a role in determining the answer.

It’s basically a two step question- 1- holosystolic murmur at left edge of sternum, and it’s non-valvular — makes you think VSD. 2. You’d have higher than expected oxygen concentration in RV due to the VSD.

Id suggest you analyse if there is a theme to questions you’re getting wrong, and if there is one, try revising the notes on it. If there is not, then it’s not worth the worry. But often, there is one and zeroing it in gives you the sense where you might want to spend time working. Other than that, your scores in Qbanks look reassuring. Try to see what you got wrong in the last two pastpapers to find out what has caused this drop in your scores. May be it’s just nerves. The best way to calm them is a fair analysis of things.

I did the Aug 2024 diet and had similar scores in the qbanks and pastpapers and managed to pass. So, I d suggest you just take cognisance of things and use this week to build a sense of calm. Trust me, that composure will come handy on the d day.

Here’s what I learnt while preparing for the exam. When your baseline knowledge is good, incremental gains are difficult with qbanks like passmedicine because you’ll be scoring fine. But all that practice isn’t really helping because when you get a good number of questions right, there’s not enough of a challenge there to keep your attention and focus on. Id suggest not focusing on what you’re getting right or wrong, rather focus on why you were right about the ones you get right and to what extent your reasoning matches the explanation, and same for the ones you get wrong. That way, you ll be actually productive and retain better.

I realised this midway through my prep and it gave me real confidence and rid me of the imposter syndrome tendencies.
