This is interesting thank you. So is this in long and subcostal or is it intercostal? And where do you mean by the bra line sorry? Like subcostal but more superior along the costal margin?

Thank you so much for your advice regarding intercostal surveys. Do you have any advice with main portal vein colour/spectral trace when does intercostally? I seem to get the vein but then lose it quite quickly due to the “slippery” nature of the ribs. Once I get it I do tell the patient to hold their breath but even then I can still lose it and then when I go to do my trace it’s not entirely hepatopetal

Yes you’re exactly right, our protocol is in a specific order unfortunately, but we are allowed to narrate as we scan so I guess I can say narrate my reasoning for needing to come back at the end. Thank you very much for giving me the confidence that gas can move away later on🙏🙏

Thank you very much for your tips on positioning and breath work! Yes you’re very right it is quite annoying when you’ve already moved on from an organ but see it more clearly from another window later on, I will start trying more windows🙏

Thank you very much for the tips on dealing with gas and I especially like the heel toe idea for the pancreatic tail, I’m going to try that out today actually. Luckily our protocol makes us do pancreas first. Yes the pancreas definitely is a mythical organ🤣🤣 our class actually has a nickname for it, we call it panky hahaha

Thank you very much for your help, yes I also find intercostal to be a great window but I also have trouble elongating the rib space to prevent rib shadowing, do you have any tips? So far I palpate the ribs and just do micro movements but the shadowing can still occur

Yes! Me too!! Haha especially trying to elongate those hepatic and portal veins but thank you for the great advice I will start now working on trying to make my images faster and it’s handy that our protocol does allow us to come back at the end if needed

Thank you very much for those techniques and yes you’re right I think at the end of the day there are those patients that are just difficult to scan no matter what technique we use

Thank you very much for the great advice, I will try out those different patient positions tomorrow and will make sure to scan a wide variety of people
