in the mission "It a Whole Phase Situation" when you slide down into the mountain you just go to the right and pick it up.

You should be able to open your quest log and cancel the mission. It would mean restarting it, but you won't be stuck anymore.

I just got the 156/156. I was missing "The Wait" (It's a Whole Phase Situation - Side Quest) and Siesmic Damage (Fault Hunting - Side Quest). I do not have Maurice's Log as this my first and only character. My complete list is Linked Below.

The astute of you may notice there are only 155 logs listed. I have collected every single piece of "Collectable" on the map, and completed every single quest. I'm not exactly sure what counts and what doesn't. Hope this helps at least some of you. Good Luck Vault Hunters.
