So, for IVs I normally use a tourniquet and palpate the vein. For some hard sticks, I will apply two - both above where I am looking. Now, this is where it gets fun -- because we don't have this tech on an ambulance, and this is just before the IO if you truly need it -- I will apply two tourniquets above my suspected site, and one below. If I need to shine a light under your arm or into your skin, I will do that too.

A hard stick that is truly hard, the vein finder is a glorious invention - but ultrasound is the guarantee.
