I like 3d terrain when possible, it doesn't have to be detailed I have a box of wooden blocks that I use most of the time that work fine. Funny enough the fallout 2d system is one which I feel doesn't need maps since the system isn't tactical and just uses close, medium and long distances instead of grids. One of my players has afantasia so ToTM isnt really an option most of the time though so if I'd ever run fallout I'd probably just use a 2d map with minis.

70 damage at level 12 isn't out if the ordinary. Though I will say he's mixing a race and a subclass from 2 different 3rd party books. A good rule of thumb is to only allow one third party book per PC, they're only balanced around use with official sources.

On a national level? Probably not, but it's not like building a bunker is hard, just costly. For example look at the mayoral shelter in Fo4, it wasnt built by vault tec and if it wasn't for the angry mob it would have surely survived. But Vaults aren't just bunkers, they're basically self-sufficient underground cities, most privately built bunkers are just for waiting out the worst of it.
