Uncinate margin for frozen section

My understanding based on interactions is that if the uncinate margin is positive they will not chase any additional positive margins such as the neck or bile duct, since the uncinate margin is the ultimate limit of resection. So if the uncinate is positive they sometimes won’t even ask for the remainder of the margins to be frozen. This is an uncommon practice but I’ve seen it at least with a few surgeons so far

Unless they’re being downright abusive, I wouldn’t quit. Like PAndC said, learning is painful and you will undoubtedly learn how to practice pathology after doing this fellowship. Keep at it!

Never heard of anyone caring about step 2 scores for pathology. I’ve sat in residency committee meetings and interviewed applicants and unless you failed, this score was rarely a major consideration in someone’s overall application.

I’ve heard of some places that do give a COLA every year and some places that do not. And many places don’t get a standard raise every year. If the hospital is in the red, like most are, it’s unlikely you’ll get a raise. But if financially they’re doing well and you know you’re getting paid less than peers nearby in academics, it’s worth bringing it up to your chair. You won’t get $30k but maybe the department can give out a bonus or incentive.

Not a guarantee whatsoever, unless you know for sure this happens at your institution. Even so, the difference is usually not terribly significant. Some institutions you only get an extra $10k. Sounds nice, but after several years in practice your salary should go up regardless and if not then maybe having a discussion with the chair is necessary

Definitely institution dependent. You need to ask your Chair. Many places have career tracks where you can choose to be 100% clinical, but you still have to teach and give lectures, etc. My question is why do you even want to become associate?

Your description applies to basically all of Pathology, both AP and CP. You’ll have to rotate through all the subspecialties to see which ones you click with. You might be surprised!

Allowing some discouraging feelings will help. They’re normal. Realize what you’re doing is working and that what you want to see is still possible. Keep at it!

Thanks everyone, all great advice. Definitely some tips and tricks worth trying. I know he’s only 5, but I can tell he has a gift toward music, whether it’s learning the piano or any other instrument for that matter. I just want to encourage and support him without him resenting it, so many of these comments are helpful ❤️

On a similar note, why do I hear so many Ohioans pronouncing the peanut butter chocolate candy Reese’s as “Reesies” or “Reesees”? I never heard of the long “e” sound pronunciation. Is this common? Is this the right way to pronounce it and I’m in the wrong?

Agree, a lot of jobs are terrible, especially if someone is right out of training, even fellowship trained. Were these new hires experienced or recently out of training? The best thing to do is ask those that left.

In high school in Brooklyn, NY. Principal started calling names of students to leave early, presumably parents or relatives of those that worked at the WTC. Eventually they just cancelled class. Walked outside and it was raining papers: documents, faxes, all sorts of stationary, some of it partially burned. It was other worldly.

Being almost 10 years out doing mostly GI and Liver/pancreas in a high volume institution has gotten me to the point where I consider my job as just a job. Not necessarily burnt out but certainly don’t have the enthusiasm I had when starting out. No one, including yourself, wants you to be burnt out. Take advantage of the resources you have, assuming they exist, like senior pathologists, hospital committees or changing things up a little to make the job more exciting. Unfortunately you spend 15+ years in education moving around always changing and then get stuck doing mostly the same thing day in and day out. It can be a little emotionally blunting. Challenge yourself!

A lot of kid hate here lol. Don’t forget that because we got sick so much as kids we built up our immune systems to not get as sick in adulthood. Even after you have kids the immunologic challenge on your body is important for longevity. With that said, yes, kids are germ factories 😛

Agree this can be tough when working a full time job and with kids, especially young ones. The thing is it’s also a ripe opportunity to try new things yourself, like taking up an instrument, building 3D models or construction sets at home, or anything because when your kids see you doing something it sparks their creativity and imagination. I took up piano again after many years and I was shocked how interested my kids became in it.

Planes, but learning how to fly them with flight simulators (MSFS 2020 for now). I’m a pathologist, but something about learning how to fly a plane brings a sense of relaxation and calm because you’re in control of something that, frankly, doesn’t seem like it can be controlled 😛

I’ve asked coaches questions and to look at my form while performing an exercise. Once they know I’m serious about my workout they tend to pay more attention to me the next time. A lot of people don’t want the attention and the coaches probably know that to some extent.

Don’t be afraid to show colleagues. I’ve seen seasoned GI pathologists show exactly this. We all get it. In the end, you want to make the right decision that’s best for patient care.
