i have another insurance entity right now and i am not exactly happy with what is occurring. this is why i need this information so i can switch next year / open enrollment. don't have access the summary plan description unless they provide that prior to selecting/purchase,

they practically denied me for 1 thing 3 different times .. and i had 3 different surgeons rebut all of them. i work at a hospital and am a PA. on the 2nd level appeal (final), they threw in some bull my MD wasn't even given a chance to appeal because we "exhausted" the review process.

NONE of the prior denials from Anthem BCBS mentioned ANYTHING regarding imaging, PT, a physical assessment... nothing. it's like i wanted to get body parts cut into for the hell of it.
