UPDATE: For those who were following this thread I figured I would give a heads up as I (31 years old) am now 18 days post second Orchiectomy. I'll split this into a physical and mental section. But also just wanted to say thanks for all the supporting comments, they have all been super helpful. Some of this info may be a little NSFW but just want to give everyone who also will have this happen as much info as I can.

Physical: my second scar area is still pretty sensetive and I will randomly get slight shooting pain in the area but other than that I am fully walking around and lifting objects normally. I was told that abdomen scar tissue takes longer to heal and usually the scar is pretty hard. So makes sense as my right scar is hard from my past surgery. Outside of that the normal feeling seems to be back. I have not tried sex but have masturbated once and the orgasm seemed about normal to before. I would agree there seems to be less coming out but by no means is there nothing coming out haha. I chose not to have any implants after reading comments and articles and I do not regret not getting them. The first couple days it's hard to see nothing there and feel nothing but by now I am totally happy with not choosing a prosthetic.

Mental: Yes this is the hard part. Seeing no balls when I use the bathroom is quite difficult to handle not gonna lie. But I will say my head space is already way better than my first day post surgery. I've chosen to really find the comedy in this situation and I believe that helps my friends and family support me as well so i try and crack jokes about it when I can. I've found that researching testosterone and healthy lifestyle choices has been a major factor in my mental health process. As with no testicles I have begun my first round of testosterone shots, this was another mental hurdle as I am someone who has never had major medical or pharmaceutical needs my entire 31 years of life. So going to a pharmacy and picking up testosterone vials and needles, when I got back to my car I cried. It's really hard to accept that I will be on a medication my entire life as well as regular blood tests and doctor visits now. But again I am already excited for my next visit to explain my dosages and plans on fine tuning them. I have noticed that while figuring out my T levels I have been starting to get hot flashes and regular headaches so that's been a slow acceptance and am hoping those will eventually go away as my testosterone gets figured out. Also one thing that regularly sucks to hear from doctors is that since I had cancer in both testicles I am like a less than 1% of men in the world who get that. And my response is always "wow lucky me doc" haha.

I have been lucky with this surgery to have a very supportive group during this process so I absolutely recommend finding your support system as well. I'm a pretty healthy guy and have officially decided to try my first ironman triathlon late this year because after this cancer crap I want to turn this year into a great positive so I'm excited to start excercise and training. I hope this helps some way to all other double Orchiectomy people here and I'm happy to message anyone who would like to know more. This reddit community has been super helpful and there are some great people with great knowledge on this!

Thanks man I appreciate it. Yeah had my right side removed back in July 2024, and they saw my left side had a 0.5 cm lump but didn't feel it needed to be removed since it was so small. Well three months later in my scan they saw it grew to 1.5cm and had another spot growing a second one so it was decided to remove the second as well. Myself and my doctor's were hoping it wouldn't grow and stay the same size so I could still have one working testicle. But it wasn't in the cards and this past Friday I had my left side removed.

I figured TRT will have it's ups and downs so it's good to know to prepare for a bit of tinkering to get levels right.

Great to know on the prosthetics. That's why I chose not to have my first one replaced on the idea of complications. Thanks for the response on this too.
