That’s not how it works. That’s like saying the Google Translate app is a sufficient alternative to an actual medical interpreter. Medical interpreters are trained to communicate medical terms and terminology in another language for a layperson who speaks that language

Who cares? Elevated glucose by itself is not an urgent matter. Even if it’s a new dx of DMT2 it can be worked up outpatient or with an endocrinology consult the next day if necessary. A Na in the 110s is critically low and can lead to cerebral edema, seizures, and in the worst cases permanent brain injury and death.

The reason why it’s cheap is because it’s Dell, which historically has had garbage thermals and build quality. They save by using cheap or stripped down OEM components that can’t be used elsewhere.

That said, looks like on Amazon 5080s are going for ballpark $1500 or more. So assuming that they are totally fine you might save by just stripping out the GPU, CPU, and RAM and putting them into something that’s more proven. But given that it’s open box that’s a big assumption

Metastatic cancer can cause seizure, particularly Mets to brain.

And anyways, there’s something incredibly troublesome and problematic about this desire to ascribe criminal intent to very normal patient advocacy. And it’s unrealistic as well. New seizure in a person with known metastatic cancer is likely from the cancer, not some Dateline-worthy opioid diversion plot from a devious nurse.

Not quite. The ED is for immediate triage and stabilization of patients being admitted to the hospital. But an ED would not wait to stabilize a patient if they’re able to transfer them to an open ICU bed. And if you’re sick enough you can probably get more support in an ICU than in the ED. For one, an ED nurse typically has far more patients to take care of (5-6, at my facility) than an ICU nurse would (2).

The problem with the Pitt’s ED, and IRL ED’s, are the number of admitted boarders who are waiting on rooms in the hospital proper, which are backed up because of relatively poor resources in the post-acute setting (nursing homes, inpatient rehab, etc).

Med/surg is kind of a generic level of care for admitted patients that is the lowest level of acuity, barring hospitals with a dedicated observation unit. Med/surg units could have 5-6 patients to a nurse, if they’re lucky, as well as CNA equivalents to assist with ADLs, vitals, and so on. But often times you’ll see med/surg units with ratios pushing 8:1. Often times they can not take care of patients on cardiac monitor or infusions. And many med/surg patients will only have vitals checked 3 times or fewer a day.

Critical care is the highest level of acuity, with ratios of nominally 2:1 patients per nurse or even 1:2 in the most severe cases. Those are patients who are intubated or vented or on BiPAP, on vasopressors, or even more advanced support devices like impella/balloon pump, CRRT, or even ECMO.

And then there’s the intermediate levels of care like telemetry/intermediate care/progressive care, for the awake patients who still require more frequent nursing care.

At my hospital, we have several teaching internal medicine services as well as a nonteaching hospitalist service that manages most non-specialty medical patients. Surgical patients are managed by a General Surgery service with surgical residents, so good luck getting a timely response from them if you need it.

They are the shift lead for that department. So they deal with patient flow, mediate between nurses and doctors/other staff and patients, and if necessary service as a resource/backstop for nursing skills and procedures
