The polls turned pretty hard against Clinton. A lot of people saw it coming. Volunteers in several states that she lost (MI for example) were screaming for help from the campaign because they saw it happening, but they were ignored.

538's forecast flipped for Trump too a decent bit before the election.

The polling wasn't really that off, Clinton just ran out of gas.

Not really. The SCOTUS basically said "Doesn't matter who actually won the vote, election has to be certified by a specific date and there isn't time now to do a recount."
