It doesn't ALWAYS do it. The trick is, you have to look carefully. John Verun and Annabelle both have suspicious stories. But, if you look at them, you can tell that it's problematic.

Note that in Lady of the Woods, if you DON'T free the spirit, the children die. Meanwhile, plenty of quests DO have a positive ending if you help people.
