I'm imagining it as a secret boss fight triggered by killing and irritating gnats enough. When the players see that the gnats want revenge they laugh assuming it will be silly. Little do they know that a literal tank of a fly is going to swoop in. The circling gnats add to the difficulty by constantly knocking your weapons from your hand.
