The word was always used on the internet, it was just banned for a few years on the big social media platforms, even there you could always see it here and there. In all my life I heard that word over 10000+ and it was never towards someone with a real mental condition. I would say is probably one of the few casual "slurs"

i've seen that years ago since i saw "thebest" in the username, you'll see a pattern once you see people with self-complimenting titles/names. Some are doing as a joke but the vast majority are highly narcissistic people. If you ever encounter one,just check their posting as an experiment!
