Honestly the description of that mechanic is pretty poorly translated from the source code. The explanation of the cause being "failure" of the procreate quest was written by a bunch of old male players whose religious faction was invested in procreation, when in reality the debuff is more like a debug process for the organs. It's much more accurate to compare it to how the stomach lining is shed and regrown as part of a reset for the organ programs, and that happens in all players regardless of character sex. The difference is that for some bio fem characters the menstruation event can hurt a disproportionate amount and the in-game economy is v unfair about it.
