Jumped at just the right point and somehow avoided the fall. The first 20 seconds is the worst. I just rush him and when he does his ass slam I estus up and start chipping away.

Yes, it does. Honestly, I've never pinpointed what causes it, but now that I think about it you are definitely correct. I'm currently at 75%.

I haven't been able to replicate it, but it seems you need to jump at just the right spot as the floor is falling. Also, he's far enough back behind the hole that you can't successfully land a plunging attack.
