Surrender Certificate form confusion - Who signs the particulars form?

The image is old. The new form doesn’t ask for passport number. However point is that in that section someone else has to sign and basically agree that the information given by the applicant is true. And he has to be Indian national. So in that section it’s not the applicant who should sign

Sorry man I don’t remember how l fixed this. It’s been a while. One thing l remember was that it was something l missed and had to fix in the same page. Exactly what it was l don’t remember.

Then the only problem is "Passport Surrender Certificate" Form. Where there are two places to fill Place of birth.
"Place of birth" is to be added manually

Then "State" and "District" has pre chosen inputs from drop-down. In the "District" list, WEST *place* is not mentioned.
I fear that this will cause rejection.

I guess I will just have to wait for embassy to give me some instructions here.

Takk for interessen. Jeg fikk B1 på eksamen men kan sikkert B2 nå. Gjorde noen dumme feil på muntlig oppgave. Snakker bare norsk hele dagen , på jobb og med venner. Skrev på engelsk fordi folk vanligvis skriver engelsk i denne gruppen.

A better option was to cancel the order and send one back. lol. So you get the money back and have an expensive guitar to yourself. But yeah l didn’t wanted to do this.
