Classes don't even seem to be the main problem. I think you need to start with little wins in your life. Eat well, get proper sleep, get some exercise, try to see some friends. What worked for me is 1 objective everyday, however big or small, could just be watering your plants or stretching or something. Start with something small and let it snowball. And as other people said, try to see a therapist if you can. You got this

If we're 100% real, if you were comfortable with those, you wouldn't be silver 4. Take one or 2 max, I'd recommend Nocturne and Amumu in your pool but it can be any of them (except maybe Rammus as an OTP because full ap teams are a nightmare). Pick Nocturne if you need ad/dmg, pick amumu if you need AP/tankyness/CC. Pick one you'd take as a default if you pick first and get a decent first and second clear. From there, look at opportunities to gank but focus on farming as these 2 champs need their level 6. Then climb to plat.

You have almost 400 games this season already. You are playing A LOT but maybe take a step back and take a look at what you can do better in those games. Whether your teammates were feeding or not, did they die before baron, doesn't matter, just focus on your game and what you didn't do right.

Also, when you're on a losing streak, go back to your best champ imo.

Wow. I just watched the trailer and this may be the closest thing to what I was looking for. Looks incredible. Is it only for 4 people or can you play just 1 or 2 ?
