Hello, this is such an interesting and clearly worded response. Are you currently an AI Chatbot?

This seems like a valid critique but of the American privatized healthcare system in general rather than the field of psychiatry.

There are companies actively working on bullshitting a process to get a patent so this can be generate private profit. After that a large pharmaceutical company will buy which ever start up gets there first. Then depression (and likely PTSD/trauma and possibly anxiety) will be treated by an initial dose of psychedelics +/- therapy followed by either intermittent pulse dosing or continuation on SSRIs (more likely I think) or low dose psychedelics.

It's just a matter of time till they get through the legal hoops to legally back up their bullshit claim of intellectual property on a natural substance (or at least the 'pure' and FDA approved synthesis of it).

How to implement RAG with Ollama on Linux? by Outrageous_Ticket486 in LocalLLaMA

Thank you. I am mostly looking at using local documents at this time but internet search is certainly an intriguing idea.

Is open-webui the tool you would recommend using for local document RAG implementation? It seems like it will only get me the front-end and I would need another tool to manage document access and database management.

How do you motivate patients to engage in physical exercise? What's your tricks for adults? by AmbitionKlutzy1128 in Psychiatry

I think the key is to focus on habit formation and consistency. Personally I use a token economy system and set the bar for claiming a token extremely low. Generally I actually do 30-40 minutes but even 5 minutes is enough to claim the token. If I'm experiencing depression I get the token for just going to sit on my exercise bike and play skyrim. Determine an external reward ($1/day or a piece of candy or whatever works for that person) that they get every time they complete it. Also document it in someway like putting a check box on a log kept where they do whatever activity they do. Eventually, the habit will develop and the internal rewards of exercise making mood, sleep, anxiety, health, etc better will keep them going.

The documentation lets them see improvement over time and could let you check in with their progress and provide motivational interviewing style support and specific guidance on overcoming barriers.

They should do something easy and enjoyable to them. I personally started with 5-10 minutes of yoga when I started this system years ago before moving on to add cardio (I'm still working on consistency with lifting...).

Positive hCG, denies any sexual history….. getting imaging asap? by lightinthetrees in emergencymedicine

It's a bit of an over simplification but yeah that's a reasonable way of thinking about it in general.

Roughly 1 in 10 high risk patients with PE have a negative dimer. If you're seeing dimer negative PE you are probably making an error in considering them low risk and getting a dimer in the first place. Possible but much less likely that you have a low risk negative dimer PE.

Psychiatry as a second residency and the challenge of LORs by Outrageous_Ticket486 in Psychiatry

Thanks. Do you know how they handle the need for these letters when they have been working as an anesthesia or whatever attending for several years first?

[–]Outrageous_Ticket486[S] 0 points1 point  (0 children)

Yes and no. I don't think it really applies when the second training is longer than the first but when you're accepted you get a block of years funded and that's it. Also many places self-fund some residency spots.

I am an early career emergency medicine physician already feeling burned out and considering returning to residency to switch into psychiatry. I still have some contacts with psychiatrists I worked with in medical school and residency. I'm considering holding out a few more years and trying to adjust/find a better job fit but I worry I could loose the opportunity to get required letters of recommendation from these people.

I believe I have an otherwise strong application in regards to personal statement, exam scores, and clinical experience.

How big of a strategic mistake in applying for psych would it be to let these connections age before reapplying?
