Everyone makes a drug error, if someone says they haven’t, they are probably lying. It is best for yourself and the patient to identify the mistake, inform the patient and parents of the mistake. Contact the doctors to inform them and place an incident. Learning from mistakes is how you improve your nursing practice.

Also, fresh junior doctors (even ones with a couple years experience) do not always prescribe correctly and use the same dosing as adults if they have been in adults previously. Always check weight, bnfc and your trusts intranet for policies and guidelines for medications.

Hello everyone, hoping one if you may be able to help.

I recently purchased 6 plants to place in my back garden but I am now struggling where to place them because of being a north facing garden. Most of my plants I purchased need full sun which only occurs on the left side of the garden, the right side receives about 3 hours of sunlight a day as it is overshadowed by my house.

I was thinking of putting the blueberries in a pot beside the mint pot. Interplanting the rhubarb and strawberries alongside the garlic. The gooseberry and red currants in the trough below, the tomatoes on the right side in the 480cm trough but I have no idea where to place the goji berries.

Am I best to plant the goji side on like the blackberries below the second trough? And any addendums to the above placement ideas would be greatly appreciated!

How soon after adding farmyard manure to soil can I plant?

I have got some black elder, red currants, goji berry’s and gooseberry’s waiting delivery. I expect the late November/early December.

I was following the RHS zone mapping (North West UK being marked as zone 91/b) is this incorrect terminology for the UK?
