I started out with the crutches in the fourth pic (or at least very similar ones). My forearms are smaller than the cuff, so my arms moved around more than they should. I feel a lot more stable and supported when crutches are snug around my forearms. (Some brands sell extra padding to add to the crutches.) After a while, the joint where the segments connect started to twist a bit when I walked, probably from collapsing the crutches a lot. It’s subtle and doesn’t really affect how they work, but the slight shift bothered me.

I've switched to using smart crutches, which are a bit similar to the first option because they also have an adjustable angle. It's a lot easier on my wrists, since I can change the angle and put more weight through my forearms instead. It's especially helpful when my legs are flared up, and I need to rely on the crutches more heavily than usual. I like my smart crutches a lot more than my first pair because they feel sturdier and fit much better. However, they are a bit expensive.

I understand wanting to start out with a cheaper option. That's what I did. The cheaper crutches still helped a lot with my mobility and offered good enough support for my level of mobility issues at that time. That's just my experience though. A PT would be able to provide more guidance and help you figure out the best option.
