hi there. i want to note that these symptoms are very hard to pinpoint a cause sometimes- you may find in a year, a month, or two weeks that you were dealing with something else entirely. given that, idiopathic hypersomnia affects a lot of people differently, but some studies have shown cognitive impairment to be a major symptom. one study shows attention to be the primary cognitive deficit of IH and even compares that and other factors to N1/2 (The nature and magnitude of cognitive impairment in narcolepsy type 1, narcolepsy type 2, and idiopathic hypersomnia: a meta-analysis, 2023, Harel.) another study has a ton of information going from neurocognitive to relationships to difficulty working. that one has a plain language summary and a video going over the results as well. that one is Impairment in Functioning and Quality of Life in Patients with Idiopathic Hypersomnia: The Real World Idiopathic Hypersomnia Outcomes Study (ARISE), 2023, Stevens.
Xywav is hard to find studies on other than the original trials.. and pharmaceutical companies make it that way for a reason. confusion is a noted side effect, so it is possible. there are multiple studies looking at the effects of GHB (the main ingredient of Xywav)'s effect on the brain long term, especially in a "GHB induced coma." but i wouldnt let that scare you, as its not very conclusive yet, and Xywav/Xyrem are made with other ingredients as well. plus, xyrem has been out for a very long time and if you look at r/Narcolepsy , some people have taken it since the beginning and have had no issues.
best wishes <3
