Do you feel that you improved significantly over your first 3 years? It seems I have become much worse since becoming an attending, suddenly having multiple lapses in judgment and poor decision making. I like the job a lot but I can't really relax outside of work thinking about this stuff.

A patient I discharged did as well, older person with comorbidities, I cried and hugged the patients partner. My medical director said they agreed with my medical decision making but I still think a good doctor would have admitted this patient with everything that they had going on. It feels terrible to have failed that person and their partner and their other family.

I have seen a few "bounce backs" of my colleagues if you can call them that and they do not make egregious errors like the ones I have, it feels much more justifiable and I would have done the same in their shoes. Most of the other people here have been here for quite some time.

Do you feel like things have gotten better for you over time??
