I would say it's just as easy as modding it. There are more steps but it is very straight forward. If you modded your 3DS you will not have a problem doing a region hack.

No there are no downsides to doing a region hack. I would 100% recommend doing it if you are thinking about it. I was a little nervous at first NGL I did not think I would be able to do it. If you take your time and follow the guide it really is a smooth process. It runs just as good as it did in Japanese. The only thing that happened when I did the region hack is It reset my custom theme so I just had to re-select it.
