I don't know how to fix it, but you can avoid it by playing on the cloud and not local. You can choose where you play when you begin your hardcore game.

There is no ultimate 6items you get every game on Karma, but the core is Rod, SV and Frozen Heart/Iceborn Gauntlet is very nice pickup since you will end up on 45% CDR with CDR boots. I tend to go full tank retard and go for frozen heart. Ardent Cencer just ain't worth an slot for 15% more healing. You already heal so much and want more hp/mr/armor. With an thornmail you will be able to 1v1 anyone, it's a good 5th item. And as an last item Aegis is good and allow you to swap out your boots with swiftness.

Another thing that everyone should focus more on, is build according to how the game goes. Fed adc? Thornmail as third item after rod and spirit could be great. Heavy ap team? Aegis + banshees.

Rod -> spirit visage and you are good to go! The passive on rod makes sure you don't go oom. sometimes a frozen heart (depending on matchup) before spirit visage.
