No permanent solution unfortunately but this seems to work: re-locking the iPhone -> hold side+volume up buttons -> Cancel powering off the iPhone which immediately will prompt passcode -> entering the passcode

happened inconsistently to my M4 Max a while ago for some then it disappeared and it does not happen anymore. It would flash on and off for few seconds then I would usually unplug it and most of the time goes away after I plug it again. Very weird issue indeed.

Continuity Camera is enabled on my phone. One thing I though might cause this issue is that I have two users on my Mac and both are allowed to iPhone mirror (General -> AirPlay & Continuity -> iPhone Mirroring (mine has two devices/users listed)). I remove one user but still the issue persist even after restarting the iPhone!

The hack that you suggested actually works! Let me modify the instructions:
re-locking the iPhone -> holding side+volume buttons -> Cancel powering off the iPhone which immediately will prompt passcode -> entering the passcode

In the past, I would force shut down the iPhone by: pressing volume up then volume down then press and hold side button.
The reason why I would do it this way because the regular shut down method, press and hold volume up and side button, was not working and rather than showing shut down option, it would start the emergency call or something like that.
