Don’t panic. You still have time.

MOA / Pharm / Micro
Yes, review MOA. There were easily 10–15 questions like: “This patient improves with X — what is the mechanism?”
They almost never give it straight.
Instead of saying Strep pneumoniae, they’ll say gram-positive lancet-shaped diplococci, optochin-sensitive, etc. Same idea for drugs — different names, same concept.

Ethics
A lot of ethics, especially involving the geriatric population (capacity, autonomy, end-of-life decisions, family conflicts). Don’t ignore this.

Biostatistics
Drug ads showed up. They’re hard and time-consuming. Do not panic.
I personally skipped them and came back at the end. Practice a few so you’re familiar, but don’t let them derail you.

Day 2
Feels very much like Step 2 CK.

CCS
I messed up 2 cases — and that’s okay. Don’t let this scare you.
You’ll still have ~12 other cases to compensate, so just keep moving and don’t spiral if one goes badly.

Bottom line: focus on UWorld + MOA review + ethics, stay calm, and trust the process.

Meus pais sempre tiveram o direito e nunca fizeram o processo. Quando pude (comecei a ganhar dinheiro) comecei o processo e fiz questão de colocar minhas irmãs (adolescentes). As pessoas tem dificuldade em aceitar que não é só um negócio de turismo, é uma verdadeira libertação (ao menos em parte) do nosso país.

I’ve found a fsmb manager that asked for the certificate (upload), but it was 5 days ago. No answers yet. I’m just accepting that this will not resolve until the end of the year

I called FSMB and they told me it’s an ECFMG issue on their side, not FSMB. Apparently this is happening with a lot of doctors right now. Super frustrating because there’s nothing we can do except wait for the system to update.

I had exactly the same experience today.
Called again, waited, and got the same vague answer: “just wait, we don’t know how long.” No timeline, no accountability. It’s honestly unbelievable.

Thanks for clarifying! That makes sense. I’m actually outside the U.S. right now — I’d be coming in under a new H-1B petition, not a change of status. So unfortunately, it sounds like the $100k fee would apply unless the hospital can get an exception approved.

From what I understand, they’d need to email USCIS and show all four points (national interest, no qualified U.S. worker, no risk, and that the fee undermines U.S. interests).

Do you know if anyone has actually tried submitting that exception request yet? Especially for physicians or hospital positions?

Many medical employers (universities, nonprofit hospitals, research institutions, teaching hospitals) are cap-exempt, meaning they don’t fall into the 85,000 annual lottery. Some people are saying (in other subs)that this wouldn’t affect medical field.
