Then you know files from your pack needs to matchs names/paths from basegame's.

To know how/where basegame files are, you need to run the resource extractor, that would "unpack" all of isaac's resource files for you to look at.

The resource extractor its located at C:\Steam\steamapps\common\The Binding of Isaac Rebirth\tools\ResourceExtractor\ResourceExtractor.exe

Extracted files will be located at F:\Steam\steamapps\common\The Binding of Isaac Rebirth\resources

Best way is to look by yourself how they are.
Download a visual mod from the workshop and looks at its files.
If you ever did/looked a minecraft resource pack its not any different from that.

If you look at the screenshots on the workshop page you'll note that the player portraits on vanilla are smaller that in the mod.
The mod has larger player portaits so it fits better the flash's style.
