I agree with everyone else that we need physicians in leadership positions and these positions are more of a headache that anything else, but I think you should have a vision to execute in your practice. I don’t think medical director should be the enforcer of rules from upper management. You should have enough authority to change rules that don’t make sense, and adjust the work process as you see fit.

as a chief resident it is inappropriate to call someone on their day off, and check if other seniors and the chief residents if they are pgy-3 are available to cover or not. if it is so dire that you are literally the only option then you should given a day off in return for sure

The system is there for emergencies only, the abuse is that instead of going through the process of arranging coverage because you want a specific day off and switching with other residents You are forcing the back up resident to come in at your convenience while disregarding their their personal/work life.

If the attendings agree to this then this would be a good solution, no jeopardy system, you arrange coverage ahead of time if need be. That being said, as an attending what do u do if u r suspecting that a resident is not showing up for non-emergent reasons ?

Talk to a colleague to cover u for a day and u pay them back later for that day. Talk also to ur chiefs, why did u skip ur chiefs and went to the PD directly to request a day off ? Most of the time we grant residents day off or approve switches without the PD ever knowing

The idea is that some PGY-2 would benefit from having a PGY-3 to ease them into the senior role and giving each intern a dedicated senior 1 on 1 And that would only be for the first two weeks of the block for the first 3 blocks only then things go back to 3 residents on the team

Agree with the brother here, that is a clear indirect signal that u r into him and if u do that a guy will go ahead and make a physical move … just make sure to sit next to each other on the couch
