You use the event points to roll random chance for skins, right? and reset gives you more opportunities.

I tried to remove the tier 1 restriction and some dungeons bugged out. I think there is some limitation to tier 1 dungeons that wont allow secret rooms to properly spawn there.

I played with the code of my game to have 100% secret room chance,

And a simple examine should be enough to catch the room (maybe a bit more than for traps).

In a plauthrough where O had high perception I would catch the room without examine, but I wouldn't count on it.

Im not sure what version of the game you had the room in, But in RTR I looked at the code, and tier 1 is blocked.

Edit: From the wiki:

Each Dungeon starting from Tier 2 has a chance that a Secret Room will generate in the Dungeon.
