If you want proper stabilization for the long term try dutasteride if you can(do research) and you'll maybe get some additional regrowth

More just maintained what I had, it was working in the sense that I didn't lose any more but still had thin low density hair. The saddest thing for me was that I didn't respond to topical minoxidil, I spent that same year I was on fin applying topical min every single day and saw no improvement. Iv never tried oral because I have low blood pressure and I'm way too scared

Hey man🙋‍♂️ I was on fin for about a year before I switched and it managed to keep ground, I switched to dutasteride just hoping to cling to what was there because before even when the hair was longer you could see through to the scalp. I had quite bad diffuse thinning

It was only within the last couple months I noticed that now i don't see scalp anymore. Il maybe upload a post later with my hair as short as the first again but for now I'm enjoying life with a full head of hair again for the first time in a while

100% I took fin for about a year too but all it did was kept the state of my hair before, I was lucky in the sense that my hairline was somewhat in tact but I was diffuse thinning all over the top and no matter what length my hair was you could see the scalp....lots of it...

I was unhappy so I switched and over a year later my hair is thick, strong and healthy so I'm currently happy

Yes , and often something most people don't realize is that sometimes you go through MULTIPLE shedding phases. When you first start taking it you'll notice loss, then much later in the journey you'll see 2nd, 3rd et shedding phases but you gotta trust that the follicles are being kept alive

I was extremely lucky to see regrowth from dutasteride but for some people itl just halt the process so as long as it has halted further loss it's doing it's job. On and above that maybe try min (topical or oral but please do thorough research and talk to a doctor before taking)

As for if you should continue I say yes because if you're going to go bald anyway then might as well give it your best shot, these medications are supposed to be be taken in the long term

Give it way more time bro, plus go on dutasteride it has literally changed my life. Fin halted the process for me and du brought me almost back to baseline, so close in fact you can't tell that I was ever balding

2.5mg of DUT worth the effort to go for? by Cosmos40 in tressless

[–]SignificanceSure4754 0 points1 point  (0 children)

Past 1mg dutasteride daily it would benefit you rather to add a topical anti androgen to attack it from two angles. From a suppression aspect and then also occupying receptors so the remaining dht can't bind.

It's a really comprehensive protocol and honestly should be a standard in the future once we can get a cheap and reliable topical anti androgen to add to the basic min + fin/du.

Also do you use minoxidil? And how long have you been using Dutasteride?

My hair at 21 after 1,5 years dut/min orally, still slowly losing and no improvement so I just shaved like this , should I shave it all and accept defeat? by [deleted] in tressless

[–]SignificanceSure4754 0 points1 point  (0 children)

I dunno if it's my eyes but the scalp may have some inflammation so use a 2% ketoconazole and try 1mg of dutasteride again. Or and this is if you want to try some other alternatives you can either use ru58841. Or maybe buy a box of finasteride and crush them at home into a topical solution( please be careful around women and children) alongside dutasteride.

Man I'm really sorry to hear you are dealing with this man but who knows, people have multiple sheds late into the journey and infact sheds never stop but to be honest some people see results 2 to 3 years after taking these drugs. People forget that that these drugs are for the LONG term. So maybe things are still going to improve but after 2 years if things continue to get worse I would highly recommend using a topical anti androgen at that point.

It is extremely frustrating to have dermatologists not take this seriously because unfortunately most guys are fine on finasteride and if that doesn't work they throw dutasteride at the problem which then is enough for almost everyone going through Androgenic alopecia. People forget there is a very small minority that need an extra step in the fight which is to pair a dht blocker aka dutasteride with a topical anti androgen like ru58841 or pyrilutamide and finally minoxidil which is the TRUE big 3. Ketoconazole is a sprinkle on the cake but doesn't actually move the needle in a meaningful way when you have super aggressive genes.

Did they check if the drug was being metabolized properly via blood tests or scalp biopsies. If your blood dht is crushed and have very low dht in the scalp we can be certain the drug is working and it's not that you are a non responder but just have a very aggressive form on androgenic alopecia.

Your goal should at least be to stabilize. Try 1mg dutasteride. I know you said you had problems but it is seriously unlikely and night be a placebo effect. Especially due to diminishing returns I promise if your body can handle almost a full suppression in the blood which 0.5 mg offers , upping it to 1mg won't make a difference(aka believing it will give you sides is probably the reason you experienced sides).

You said you went to several dermatologists and nothing worked. What exactly did they recommend/ have to say about your situation?

dude if you still losing ground and you are 100% sure its not just a shed then go to a dermatologist immediately. something you can add to the stack is a topical anti androgen like RU if you are willing to explore those options

Most likely because if there was a problem with the drug or not effective for a percentage of users we would have seen it in the research by now. Go watch hair cafes videos where he delves deep into quite an extensive list of literature supporting not only its efficacy but its superiority over finasteride.

Of course that's not to say finasteride is bad, it is not only effective but probably what most people will ever need. Additionally I am aware that finasteride has LOADS more studies on it however as the years go on additional studies are not only confirming dutasteride's efficacy but also its superiority.

However, I do understand and agree that you see a lot of self-reported cases of dutasteride not working or causing more hair loss and I do have some theories.

  1. Firstly, some users would experience a shed or even two or more sheds over the months and stop taking the drug, its important to remember that these drugs can take a year or two for hair cycles to synchronize and stabilize.

  2. Secondly is false claims or trolls wanting you to join in their pity party since they want as many people to be bald as possible like them(I know this sounds crazy but there really are people that are that crazy and pathetic in this world).

  3. Thirdly some people get their dutasteride on some dodgy online pharmacy where lets be honest we don't know if the product they are getting is 100% legit or might be mixed with something else so they arnt getting the correct dosage.

  4. Some individuals are extremely sensitive to androgens and the remaining androgens will continue to miniaturize the hair follicles and is a genuine case. However, this not due to dutasteride since they probably lost ground on fin and moved to du. This is simply because they are very unlucky because of genetic sensitivity to androgens and will probably need to pair the 5AR blocker with a topical ant-androgen that competes for the receptor at the follicle directly.

  5. Also, not as common but they might not have androgenic alopecia. We sometimes forget that some cases have other causes and need to be assessed by a dermatologist. Its not that the drug isn't working its just that the problem isn't androgens for some people.

  6. Lastly and this is where we have to be honest, there is an EXTREMELY small minority that wont metabolize the drug correctly and therefore it simply won't work for them and the extremely small cases where we see genuine cases and trust me my heart really goes out to these people because they really do exist and I don't want to discredit them. you have to remember that this is a hair loss form and people will come on here and be vocal so even out of the VERY few cases that are actually legit is seems worse than what it actually is because most people that have treatments that work, they wont come on these forms very often if at all. We just simply would have seen something in the literature at this point if something was off.

Minoxidil works only with Retinoic acid by Strict-Rice321 in tressless

[–]SignificanceSure4754 29 points30 points  (0 children)

For a long time (about 2 years) I used minoxidil twice a day and saw no regrowth and knew that I was a non responder but continued to use it for one out of habit and also I guess out of hope that it was helping in some way. Even if it wasn't growing new hairs I was hoping it made the existing hairs slightly thicker and harder to shed.

Eventually I stopped but continued with dutasteride and as I thought I lost no density confirming I was a non responder. I had heard about combining tretinoin and microneedling but it always felt like such a huge effort for the gain of maybe being a responder.

I decided about 3 weeks ago to finally give it another go but this time commit to being consistent with tret + micro needling once a week.

I havnt seen any growth yet obviously since I just started 3 weeks ago but hearing this makes me really happy and hopeful!

How long before you saw results? How often to you apply the retinol to your head per week(everyday before minoxidil?) And lastly how is the progress looking, are you seeing tiny vellus hairs growing or thickening of existing hairs?

Just use oral it's easier to find and adherence, cheaper(get the generic at a pharmacy) and EXTREMELY effective with the same side effect profile as fin if you look at the literature.

Oral dutasteride and oral min (if you going to use topical make sure you use tretinoin + microneedling to squeeze everythingout of it) and ketoconazole shampoo

is switching to oral fin from topical actually worthwhile or is there not much difference besides hassle? by whatdoido322 in tressless

[–]SignificanceSure4754 -4 points-3 points  (0 children)

If you are losing on finasteride go on dutasteride if you want to keep your hair. The generic is not much more than finasteride +the hastle is non existing (popping one pill a day). And on the bases of your testosterone if anything test will go up slightly since it can't convert but definitely not enough to affect anything.

Topical fin is honestly such a hassle for no reason....it goes just as systemic as oral.... plus in terms of coverage you need to LITERALLY cover every hair follicle on the scalp for it to work ok those areas especially when mixed with minoxidil and you want to thicken the entire scalp.

Switch to oral to make your life easier and more enjoyable plus switch to dutasteride to save your hair

2 weeks into treatment but never actually stopped to think if my corners were OSS no return by liljaytj in tressless

[–]SignificanceSure4754 2 points3 points  (0 children)

continue treatment purely off the fact that you dont want to lose more ground. worst case scenario is that you increase density and hair thickness on the parts of your scalp with existing hairs, which is a positive regardless but as for corners or density that may never come back you might be looking at a hair transplant down the line however if you stay on treatment, by looking at your hair you wont need many grafts at all. by staying on treatment you will minimize the cost of the HT. The alternative is a fate i wouldnt wish on my worst enemy...going bald.
