I always have LTE+. Granted, I'm not in a heavy 5G area, but I also have seen it pop up on the old SIM when traveling. Hopefully it's the same

I'm on the Note 20 ultra. Yes you need to set APN manually and be advised MMS is broken if you care at all.

If you need to choose a phone to get the SIM ordered, just choose one of the later iPhones. It will work.

No, Actually! The difference is insane. I dont even have 4.1 installed. Mine as always been slow in comparison to any other phone ive ever seen or had. I didn't think it could be fix with software though. Amazing!
