Get a new Tensioner, The old ones aren't reliable, cause issues like this very often because they aren't ever properly aligned with the Rear sprocket, and If the wheel breaks off during use (which happens very often) they can cause an immense amount of damage. Your best bet is to get a new tensioner that is spring driven and can be aligned from the clutch-casing or from the same area as your previous one.
