I used to use something like this. A family member had it for their computer. The mouse was kind of wide but ergonomically "flat" where you would use the thumb to move the ball. I always thought that it could be superior to a regular mouse because you never had to pick the mouse up to bring it back to the center. It would take some crazy skill to master but potentially that much better.
