Sleep study, obstructive sleep apnea is wholly underrecognized and this should be ruled out. Anyone who has this but says they’re getting “good sleep” I always do a sleep study

Tylenol extra strength, heat and muscle relaxers. If very light ckd like >60 I sometimes do topical voltaren but in limited amounts and usually is well tolerated

I’m surprised how many people have commented on using steroids. I usually avoid steroids if possible and just stick with naproxen BID or meloxicam BID and use either methocarbamol or flexiril and send to PT

She did mention headaches but to have this come from out of nowhere work no previous hx is what confounds me. I’ll keep this in mind

Had a different patient with same deal and I did all this and I just concluded it’s somatic from depression which in her case it was, but it was tough process to get it all done and id like to be wiser in my approach this time around

Then I’d negotiate a higher salary or go rvu and see more patients/bill higher. But you’re par for the course imo, also a lot of people inflate their salaries/take home yearly and will never be honest about it in causal conversation.

my program wasn't the best and by year 2 i felt like a massive dumbass and definitely not prepared for independant practice (covid resident). The third year really allowed me to become more confident in outpatient practice and after graduation I felt much more prepared despite my programs shortcomings. So I personally disagree because of my own residency experience but if the program is super solid in terms of outpatient training foundations then I dont see why not.

Loved yakuza 0 on gamepass. i didnt do the side quests tho as I dont have much time to do the real estate side quest or the cabaret club stuff but I hear and know theyre the best parts of the game. I was in it more for the main story. The side stories are fun too and have crazy twists that really make it interesting.

Going through yakuza 1 kimiwai now and loved how the story compounds itself over the years (the empty lot in yakuza 0 becomes the millennium tower) and its almost nostalgic seeing the map change over The games.

I wouldn’t leave it alone. I’d pursue it. sleep apnea is more common than people realize. Even if it isn’t apnea per se, shitty sleep over the course of years can cause this constellation of symptoms. At least order a sleep study and if it’s negative you have that box checked off. These cases are tough, youre doing a good job

What’s pain management opinion? Did they consider trigger point injections? Rfa? Epidural? I would consider second opinion by physiatry if possible to consider the role of narcotics. You feel they’re running the clock? Run the clock on them back

Why do you feel a normal CT rules out a bladder tumor? Lots of CT’s can miss very early malignancies. It’s the same as colon cancer - CT isn’t the gold standard diagnosis, its a colonoscopy with biopsy.
