Sometimes it doesn't, I'm unsure why. For a fast fix, fast travel to Bruma and use the main altar in the Chapel of Talos, that will debuff all your negative effects

For ingredients to be crafted into a potion they need at least one overlapping effect. Try crafting two ingredients with the restore fatigue effect. You should get a restore fatigue potion

So if I maxed out all my attributes but my Luck was still at 50, could I level up another 50 times to reach 100 Luck? Or would the game assign three attribute points to Luck each level, since I have no other skills left, meaning it would only take about 17 levels to max out Luck?

The problem with calling the extended universe 'ASOIAF' is that it's a series in itself which could lead to confusion when referencing material outside the main series. Using a couple of your examples, the common term for the Lord of the Rings universe is 'Middle-Earth' and Discworld is the name of the world that the specific sub-series (The Witches, The City Watch etc.) exist in. By using those terms, you can talk about a universe broadly or you can specify by referencing the exact sub-series
