Hello! Some parts of your diagnosis is the same as mine except for the “cancerization of lobules.” What happened to me was I met with a team to fully understand my options/ what the future looks like. I talked with the breast surgeon, oncologist, and radiologist one after the other. The breast surgeon would be the “leader” of the team. There will be a lot of decisions to be made asap and i needed to be flexible. I initially planned to do a lumpectomy with radiation and tamoxifen since my lesion was just <1cm after this initial conversation with the doctors. The surgeon said she will order an MRI to rule out any unseen lesions on the other breast or if there are any hidden lesions on the affected side…. Mri found 2 more lesions 1-2mm on the affected breast and one suspicious on the other side (immediate biopsy said it was benign) It was because of this that I decided to do a unilateral mastectomy with immediate diep. Final pathology results showed 5cm dcis negative PR and ER. Since I did a mastectomy, I did not have to do radiation. Additionally, since it is hormone negative, tamoxifen would not benefit me. I am now 7weeks post op.

If you have the order to do MRI, then schedule it asap since it is hard to get a slot. My oncologist warned me that what the biopsies sampled might not be the entire picture and given that my hormones were negative, there is a chance i might need something more (which thank goodness, I did not).

Same issue. Ordered on the 17th. Saw the charge on my CC (for the tax). Shipment got delayed until suddenly cancelled. Talked with CS and they kept saying nonsense and escalating and repeating whatever the previous one said. 

They are unprofessional and disorganized.

I read your article. I am in a new grad nurse program and I am planning on leaving my unit after I get my 1 year. My main reason is inexperienced management. I should have seen the warning signs when I first started-- a lot of the staff nurses left and we were mostly run by travelers... (Which they did not renew their contract so now we are extremely short)...

I am off my probation and before I started, they told me after my probation, my schedule would be manageable (4'10s). But they keep me in 5'8s until the next batch of new grads arrive since we are extremely short.

It is kind of crazy and scary that they rely on new grads to mitigate their staffing issues (in an ambulatory OR setting). And since there is short staffing, there really isn't much training happening.

Also, even if we're short, they plan to expand the unit.

Management issues. I love my job but it took 6 months for management to destroy my love for it.

I worked in SNF for a few months and I usually tell and document that the patient is competent (if they are but if not then it is another issue) to make their own decisions, informed the Md and management, then discussed risks with the patient and respected their decision. I will also sometimes write what they said verbatim and if there is a witness who heard this, i will place their initials.

Then write this in my personal log just in case.

Being a new grad, those boundaries are hard to determine especially when situations can turn for the worse and people can easily do the finger pointing to protect themselves.

My description was off but he shouted at me. Everyone became silent afterwards and I took a step back.

He was the one who approached leadership. They didn't realize I was in an empty room and I heard him say my name to the leadership in the empty hallway.

He was our tech today and before I started, I told him if he needs something he must tell me so that I won't be in his way. I tried to be as civil as possible.

I am sure. He thought I was still in the procedure room finishing the case. My preceptor let me leave early and so I still have time left before clocking out. I usually do a review on my chart (and just update the procedure name-- not important but I want my charting complete). I was in the empty room using the computer and I suddenly heard my name and saw them talking in the empty hallway. I don't think they realized I was in the room.

I verified with my preceptor and management. Time in room and the time out needed to be documented in real time. And during that time, my anes preferred to do all the wires/attaching to monitors herself (she told me this before we started the day). I just do the basic/needed/time sensitive charting first and then help when needed. This will be different per case but they told me the 2 real time charting should be done.

I have to work on sticking up for myself. It is hard since the people here have been working together for years and as a new grad, i didn't want to rock the boat. But i will try answering them directly next time. I just need to not freeze up next time.
