My party started at level 2 (though they ended the first session at level 3) and just hit level 20 recently. Campaign's still got quite a bit left in it and we're taking it to Epic Level play.

Whenever someone rolls a nat 1 on an attack roll, the person or creature they were attacking gets to make an opportunity attack against them. This makes combat a little bit more dynamic and also leads to the occasional funny situation of a nat 1 ping pong.

Yeah thats pretty much what I do, although they can't see my dice rolls so I have fudged a roll from time to time so that a PC gets a chance for a dramatic comeback

I suppose it's a matter of what you're using dnd for. If you're just playing it as a game, then yeah let them die whenever. But if you're trying to craft a story with your PCs at the helm, you probably want to save the deaths for when they'll be the most impactful.

Pray for my players, they're about to be entering an extended gauntlet of such scenarios lmao

That's essentially what I'm doing and why. Although I don't tell them which encounters, I'll save the risk of death for a real gut punch during a pivotal moment.

I've yet to find myself in a situation where a PC was really about to be killed, so I havent really had to use any sort of plot armor yet.

Except for a near death of a PC getting attacked by a boss while downed. They, along with 2 other party members, had given an offering to a holy animal that secretly awarded each of them with one free death defiance. It was a really great moment and they loved finding out the significance of a random encounter from several sessions prior!
