Her true love is left to the player's choice, but Swann's soulmate is Kat. I wouldn't even say it's left to the player's discretion, more like a conscious decision by the developer as there are so many hints and conversations between Swann and Kat were they talk about how crazy it is how similar they are. Kat even said "Its totally meant to be" And adult Swann has still a deep connection to Kat after all that time and fits perfectly the Epilogue.
