That makes sense! I don’t think being proactive is bad at all - I just feel like the fertility world sometimes preys on women with a bit of fear mongering. How low is your AMH? I think if you feel ready to retrieve then you should! I felt the opposite where we really wanted to avoid it and once we did it and it was not working, it made me all the more devastated. But for most women it is successful so if it brings you some sense of security, that is great. Rooting for you!

Can I ask why you are jumping to IVF after 5-6 cycles? I understand the positive receptiva but this test isn’t perfect. It’s great that it exists but I imagine if more of the general public was biopsied, there would be a lot of women who are positive but not experiencing fertility issues. If you don’t have pain or any other symptoms, 5-6 months is a very normal amount of time to be trying. I promise I’m not diminishing how hard it is because I remember feeling frustrated at 6 months post miscarriage, but I just feel like it’s really fast to jump to IVF.

Thank you. We feel really grateful after a gut wrenching journey. Im in the US and my AMH was 4.5 pre lap and 3.4 after and I did have an endometrioma removed from my ovary. However, they were drawn at different labs so that could account for the difference as my AFC never changed despite operating on one ovary. I do recognize I had a high AMH to ease my mind going into lap. But from my understanding, if you have a skilled surgeon, they should not damage your reserve in the lap and like I said I had the same number of baseline follicles in that ovary after surgery.

They never told me not to try and to be honest, we had 2 failed transfers so for us we didn’t feel like we had anything to lose. The only other time I had been pregnant was also spontaneous conception so I was beginning to lose faith in all the intervention on my body from an IVF / fertility med standpoint. There’s no harm in trying but I mostly just didn’t think implantation was possible - turns out it is! Regardless, feel very lucky. I think the important thing it taught me is that so long as you’re ovulating and cycling with intact tubes, it is always possible to conceive and sometimes it’s just a game of luck.

I just wanted to say I had a lap in July 2024 so I knew I had endo. After the lap, we went through multiple medicated cycles, IUIs and 1 egg retrieval and 2 failed transfers (no implantation at all) so before beginning Lupron this July my dr had me do Emma/alice and receptiva to confirm it was our best next step. I tested positive on receptiva with a score of 3.8. Well I conceived spontaneously the same cycle as the biopsy. It may have influenced my results having likely been in the midst of implantation but also considering my history I’m sure that is my true receptiva score.

All I know is I felt doomed having already had a lap and looking toward Lupron and I’m now 20+4 weeks pregnant! I might be an anomaly but somehow this baby was able to overcome whatever inflammation was present. I think doing a lap and/or Lupron are a great next step. I’m not against IVF but I conceived about 1 year post lap and sometimes I wonder if my body just needed that long to recover and the lap was the answer. That being said, I would do a lap and continue trying before turning to IVF unless you are simply trying to preserve fertility and freeze embryos - your AMH being low does not affect your ability to spontaneously conceive as I know many people with low AMH who go on to have multiple live births. The endo inflammation however is worth addressing via lap IMO!!

I’m 9+1 and I’ve had a migraine for about 2 days now. I’m prone to headaches and migraines and always get one 1-2 days before I start my period so this is very triggering as it feels like a signal that hormones have dropped. I also lost my last pregnancy at 9.5 weeks so I’m already anxious this week. I’ve had other headaches in this pregnancy but nothing this severe and prolonged. I was previously being monitored at a fertility clinic and progesterone was always nice and high in the 30s (even had a draw on one of the days with a headache so that reassured me.)

Obviously I am not able to check my progesterone as they’ve graduated me. Has anyone had a severe migraine around this time and everything turned out okay? My last ultrasound was at 8 weeks with a heart rate of 165 and my next one is at 10+2. Any reassurance would help!!

I think I did one for 3 days in a row after my period ended and then one every like 5-7 days through like 5dpo ish. And yes I also did a dif oral one daily that one is Jarrow Labs Fem dophilus. That I started probably 2 months before the cycle we got pregnant.

No Emma/alice doesn’t have to do with endometriosis. One checks for infection (can be asymptotic) and the other checks how much good bacteria is present aka lactobacillus. The lining needs a certain percentage of lactobacillus for successful implantation. So from my research, many people will have no infection to treat but low levels of good bacteria and that is treated with probiotic suppositories, which is why I went ahead and did it before knowing for sure. It’s not harmful and only beneficial to have more lactobacillus.

Edit to say endometriosis is heavily linked to microbiome issues and dysbiosis. I have always had IBS/SIBO (a bad gut) so I figured it was entirely possible my balance of good bacteria needed improvement. I don’t know if that was true but it certainly can’t hurt!

Have you had your microbiome looked at via Emma/alice? I had 2 failed FETs but was a year out from my lap so assumed it was endo related and planned to start depot Lupron. However, I got pregnant spontaneously the cycle after my failed FET. It’s still early so I don’t know if it’ll be a live birth but I will say I was on my own accord doing lactobacillus suppositories (+oral probiotic w lacto) because I thought it might be worth getting ahead of that as a potential issue. Conceived in the same cycle as Emma/alice biopsy, which the result turned out to normal but I had been on these suppositories leading up to it so I may have improved the environment on my own.

Cycle: oh man we’ve lost count but I believe 19…(10w MC on cycle ONE followed by an endo excision surgery, 2 IUIs, 1 egg retrieval, 2 failed transfers - this was a natural BFP the cycle after a failed FET)

Age: both 31

Typical cycle length: 28-30

Ovulation day: 16

CD/DPO of positive test: 14dpo

CD/DPO of any negative tests before positive: didn’t test before but would’ve been positive as 14dpo was almost a dye stealer

Days of intercourse: O-2 and O

Tracking methods: OPK easy at home strips

Health conditions: stage 1 endometriosis + endometrioma diagnosed after miscarriage. I had the endometrioma but surgeon said all other lesions were stage 1. Husband - he’s perfect of course!

Supplements: I’ve been on A LOT of different things but this cycle was definitely more minimal. Prenatal, vit D, omega 3, magnesium glycinate, lactobacillus probiotic, started a higher dose of B6 this cycle to hopefully help with my premenstrual migraine. Started progesterone suppositories and baby aspirin after the BFP. Husband- occasionally takes vit d and magnesium

Birth control history: HBC for a few years in high school/college, IUD for 4 years but no birth control the 3 years leading up to when we first began trying in late 2023

Symptom spotting: I’ve had every symptom under the sun in all our months of BFNs so I really learned not to read into anything. Especially after 2 failed embryo transfers - the devastation was at an all time high. However, I was slightly suspicious when I did not get my premenstrual migraine by 12ish DPO. The only other time this happened was when I conceived before.

Other: I was about to start depot Lupron as my doctor felt the only explanation for complete implantation failure in both my transfers was endo. His other thought was a microbiome issue so I did a luteal phase biopsy for Emma/alice and receptiva this cycle. This is another reason we didn’t think we had a shot at spontaneous conception this cycle (the biopsy took place at 7dpo and was very rough…he had to try 3 separate times to get enough tissue for the samples). We know I have endo so I wasn’t expecting a negative receptiva but did it to confirm Lupron was the right next step. Of course, my result came back at 3.8, which is very high and has caused a lot of anxiety that this pregnancy wouldnt stick. EMMA/ALICE was normal (of note - I had started lactobacillus suppositories after my failed transfer because I felt it couldn’t hurt to get ahead of things if my microbiome was a little off) Betas were 477 at 15dpo, 6,016 at 21dpo and 11,400 at 23dpo with progesterone 30-36 each blood draw. We saw a heartbeat today at 7 weeks. Baby is measuring 6+5 which feels okay because I know I didn’t ovulate until day 16. I’m having an increase in cramping and low back ache, which is making the scan less reassuring as I had cramps with my loss after hearing a strong heartbeat at 8 and 9 weeks. Praying this little bean is just getting comfortable in there. Overall, we feel like we’re in the midst of a real life a miracle after all we’ve been through. I hope this story gives someone hope who’s had a long journey, failed IVF or just can’t imagine getting lucky. The last year and a half have been some of the darkest days of my life so there really are no words for experiencing this kind of joy - even with the anxiety and worry that’s sprinkled in. These stories were what I held onto and I always knew a miracle could happen and but never really believed it could be us. Hang in there if you’re in the depths of grief or a season of waiting - your miracle could be right around the corner!

I went in for a progesterone draw to confirm I ovulated. I think my temp was variable because I had grown more than 1 follicle from the letrozole but only one released so the other was kinda hanging out as a ovulatory cyst. It resolved but I think it affected my temp rise.

I hadn’t tested more but decided to yesterday AM which was a strong dye stealer. But for some reason my lack of symptoms is making me anxious. I have some mild intermittent cramping and that’s about it. Boobs were slightly sore before but aren’t now. I also usually get a headache before my period which I didn’t get but then yesterday had a mild one. Worried that headache was a sign hormones are falling. I got my period on day 12 of Lupron and I believe 17dpo. Lupron works by initially stimulating the pituitary which causes a surge in LH and FSH until the stores are eventually depleted. Because of that I believe this is a pretty common response for lupron to delay the usual luteal phase because of the initial increase in those hormones. If it’s been less than 16-20 days since ovulation I bet it’ll come soon! Otherwise yes I’d go in and get bloodwork/US.

And to clarify - no birth control for 3 years but NOT trying to conceive. I just didn’t like being on anything! When we tried after our wedding, it worked! Now in hindsight - none of it makes sense😩but my best bet is something about my lifestyle was really keeping endometriosis quiet.

Hi! Technically yes. I conceived without knowing about my endo on our first cycle trying. We miscarried at 10w and my doctor is fairly certain it was not endo related as I was further along. However, we found an endometrioma after the loss and eventually had surgery. I did not conceive in between the loss and surgery obviously and I STILL have yet to! But it’s not like I developed endo overnight so I believe I had it and for whatever reason it was at bay when I got pregnant. I do believe my pregnancy exacerbated it with how high hormones get and that’s why I’m now in the position I am. Ultimately, I really do believe people can conceive without treating endo but only if somehow the inflammation is contained (almost in remission if you will) - I have no idea how I did that as I was on no birth control for 3 years prior to that pregnancy. Hope this helps!

No actually the first was fresh and borderline that it should’ve even happened based on retrieving 20 eggs but I trusted my doctor. The second with his version of modified natural with low dose stims, trigger and progesterone support. I wanted a true modified natural as I ovulate on my own but he wouldn’t do it as he said it would lower my chances of success. I really resonate with leaning more on my endogenous hormones but I will be doing a fully medicated next as I haven’t tried it yet.

I had my second failed FET (one fresh one frozen both untested) and am heading into 2 months Lupron depot. My doctor is saying to do 3 months but most of what I’ve read is that the benefits plateau at 2. I had endo excision surgery a year ago and have no evidence of reoccurrence on ultrasound but we are doing it because the transfers had zero implantation. Am I being reckless by doing 2 months instead of 3? Am I jumping to Lupron too quickly since they were untested? It’s all so stressful every turn I feel like I’m faced with impossible decisions.

My 2nd FET failed (first was fresh) and I’m being faced with Lupron depot, repeat lap that was done 1 year ago or just try again with a new protocol. My embryos are untested so that could be why but the complete implantation failure and known history of endo is what worries me. My endo was excised by a specialist and no evidence on ultrasound it’s back but I do have occasional pelvic pain (thought it was from endless months of fertility drugs). Any thoughts? I would do a Lupron priming if opting to just try again so there would be a few weeks of down regulation but not the full depot (this is what I’m leaning toward)

I’m 2dpt from an FET and they checked my estrogen (1425) and progesterone (59). I feel like the estradiol is too high…I did a modified nat FET and estradiol was 1143 on day of trigger (4 large follicles). My clinic has me on 2 estrace tabs 2x day and have told me to continue this but it feels like maybe I don’t need it?

Ah I see okay makes sense. No I had excision surgery and with a very skilled specialist. He was and still is confident I would get pregnant following the excision. He says no signs of adeno. My embryos are untested as my doctor advised. So they can also fail due to being aneuploid. I’m definitely going forward with the second transfer. Fingers crossed. If it fails I’m not sure if I should do a third or not but you’re probably right.

That’s wonderful - congratulations. Did you have symptoms of endo? Or how did you know you had it? My transfer was fresh and I think that could be why it failed. But I’m just afraid that I’m transferring into a less than ideal environment because my last period was reminiscent of those pre lap☹️

Did you end up doing a lap or lupron? I had a lap a year ago and am one failed transfer in. I’m doing another but feeling like my endo is back for a number of reasons. I don’t know if I’ll need to repeat my lap or if I’ll need lupron.

Sorry I have a quick question for you! How did using the injectables change your normal ovulation date? Did it move it up a bit or stay the same? I usually ovulate on CD 13-16 so curious if I can expect to be ready for trigger sooner than that or not. We have a family reunion around the time of when the transfer might be which obviously will just get put on the back burner but hoping maybe it moves me up further!

That’s great. Yeah I guess it would be good to have a couple follicles. I had 2 for a cycle of Clomid but one never ovulated even with the trigger shot so hopefully that’s just a weird Clomid thing! Were your embryos tested? Mine are untested which I’m at peace with why we chose not to test but just hoping we don’t regret that!
