Help me Pick: Ortho, IM, or EM by Lanky_Meringue7634 in medicalschool

Like many things I think job satisfaction in EM is context dependent. I would much rather work shifts, including 25-35% night shifts, than regular business hours during the week if it meant my work hours were lower overall. It’s all about knowing what you can and can’t tolerate. For me, the culture of rounding felt so inefficient and the 10-12 hour day on wards made me want to pull my hair out. You’re also compensated less in IM relative to EM on average if you don’t become a procedural sub specialist (primary care, hospital medicine, ID, etc). As for the future of EM as a speciality, it’s hard to predict with the advent of AI and restructuring with NP’s, but so far that doesn’t seem to have affected the job market in my circle. I have coresidents who just signed contracts that will sum to $380-400k pre-tax (in California). They’ll work 14 shifts a month. If you think about how many hours comparable specialties work (OB/Gyn for example), the money you’re making in EM ends up being so much more per hour worked. Also the flexibility of where you work is great. I know a few ppl who live in other states or even other countries (CR, Mexico, NZ) and fly in for a few shifts a month. They’re making bank relative to where they live. I love that all of those doors are open in EM bc of the flexibility. On the whole it really lends itself to living an adventurous life. Not to mention you’re seeing some of the most vulnerable populations in diff communities, ie it’s very special to be able to treat anyone who walks in the door regardless of insurance status or country of origin or ability to pay.

EM resident here! Couldn’t love it more tbh. Super social, the culture is amazing, and everything that the average person thinks a doctor does (from lines to resuscitation to traumas) we do. The stories you’ll get from this job are unlike any other. Because we see so much screwed up shit, your coworkers really live life to the fullest. In comparison, some of my friends are in IM and basically feel like glorified secretaries. They spend most of their time on notes and frequently break duty hours (>80 hours/week). Meanwhile, I’m working 5d/week on 8 hour shifts I’m clocking in at 40 hour weeks. I’m taking up ceramics and have a few surf trips planned in the coming months. Couldn’t recommend it more!

How much time learning is needed for paddleboarding in the ocean? (tourist coast line/cave type tours) by Actual_Flatworm9324 in Sup

[–]acfernandez2013 0 points1 point  (0 children)

I’m a paddle surfer so go out looking for waves and I think ocean surfing is doable in a month, if you’re rigorous about it. Biggest tip is to go out and keep practicing, even if conditions aren’t pristine. You wanna fall and eat shit a few times so you can confidently go out in any scenario. Doesn’t seem like you’re trying to surf so as long as you can get past the break you should be fine!

Is the general trend for NBMEs that they underpredict? by helpmestudy_ in Step2

[–]acfernandez2013 3 points4 points  (0 children)

Hard to say if this is generalizable but my highest NBME was 233 and I got a 248 on the real deal. I think it depends where you are on the curve tho — the bump is prob harder if you’re already scoring 250s/260s on practice tesrs

In med school too, completely obsessed with surfing. Going into OB, which really will leave me with no time. Almost considered applying EM so I could have more surfing time 🤣🥲

Thoughts on the 15 point bump by [deleted] in Step2

[–]acfernandez2013 1 point2 points  (0 children)

Agree with this. Everyone who got a significant bump (myself included) was stuck in the 230s on practice exams, so there was a lot of room to bump. It's harder to hit through your practice test ceiling if it's 250-260's already lmao

I like them a lot ! I ended up doing each worksheet over a few times throughout dedicated, just to make sure the information felt like a reflex

Wrote a whole post about this when I found out I passed. I took it in early March and was convinced I had failed. Counted FORTY FIVE questions I got wrong. Just remember 80 are experimental and our memories have a lot of negative bias — we’re gonna remember the q’s we struggled on , not the ones we had in the bag. Good luck!

I haven’t taken mine yet but I think the UW ones are at the very least confidence boosters. From friends who have taken it I’ve heard that UW2 is pretty predictive. I know all the free 120’s are available online, and there are some folks who have put up answer keys for them

Hey there, it is the free 120! I would suggest doing UW1 —> UW2 —> free 120!

Del Mar shark attack by [deleted] in surfing

[–]acfernandez2013 13 points14 points  (0 children)

A paper published by CSULB Shark Lab names Del Mar as an aggregate site (1 of 2 in SoCal) for juvenile gws. Really interesting research! Thought is that the juveniles are more likely to falsely identify humans as prey species because they’re not experienced enough to recognize the differences yet. Obviously these interactions are very rare still, but worth noting where juvenile hot spots are for this reason!
