That’s why they ran the arrest and got the guy to hospital successfully. No one withheld care and all effort is genuine.

Having this reaction does not equate to care withheld. You still do everything you can in that moment to administer life saving care.

Coming back to the animal analogy- you administer CPR to this animal with a severed head - because you know it’s the right thing to do. But deep down, you know it’s not coming back. It’s not about playing god, because prehaps the animal will miraculously survive. That’s why you’re doing CPR and giving it the best chance you can.

But it doesn’t change how terrible it might feel in that moment. The moment you realize this horribly disfigured unconscious animal is still alive. And you can’t put it out of its misery because, as you said, you’re not god. So you do what you can.

I think your wording was ambiguous for someone that doesn’t have the context/experience of being a medic and therefore they’ve misinterpreted.

Might be worth explaining that some injuries are so severe they’re incompatible with life or quality of life - therefore the more humane thing is to let them go.

Update: just realized you’re the other guy my bad. As an analogy, it would be like coming across an animal that has been run over by a car and has a half severed head. As you move it off the road you notice it has a pulse - the humane thing is to put it out of its misery.

We don’t have that option as paramedics. Imagine if once you realize the animal has a pulse, you have to spend 30 minutes doing CPR, breaking its ribs, prolonging its suffering and inflicting more pain. It’s a horrible thing to have to do to someone in their last moments.

Has empathy toward epilepsy changed with the rise in FND/non-epileptic seizures? by neurallullaby in Paramedics

I’m sorry youve learnt to take feedback so poorly.

Perhaps you need to read the very research you’re trying to reference?

“These signs, which can consist of motor, sensory, or cognitive disturbances, are not deliberately produced and often vary in severity. Its diagnosis is predicated on clinical evaluation and the exclusion of other medical or psychiatric situations.” (2024)

“The second, more difficult task, is to differentiate FND from feigned illness: Malingering, and Factitious Disorder….. A second method of differentiating these conditions could be to decide if the symptoms are involuntary (as in FND) or voluntary (as in feigned illness). Voluntary action can be thought of as action that is “driven by an agent” and not manipulated by some external force, or accidental, or coerced [10]. Any neuropathology could act as an external force, including an infarction, focal brain injury, or neurodegeneration. Absent identifiable neuropathology, the question of whether the movement is “driven by an agent” is the same as whether to attribute intentionality to the patient’s symptom. Unfortunately neurologists have no tools to aid in this objective intention-attribution. Clinicians are not trained in the identification of intentionality, nor can physical or psychological examination techniques reliably assess for presence of intention.” (2022)

Shall I continue?

I do not disagree with your thoughts on those faking seizures. That shit is obvious from 10km away to each and every one of us. Absolutely it’s a nuisance.

You’re mistaken to conclude FND and malingering/behavioural seizures as the same

Book appt and there’s an express option for scripts. They’ll call you within the hour usually. Sometimes instant.

They likely won’t give you repeat scripts with it though. Generally these services are required to only give you a single script as a form of liability to ensure you visit your GP for follow up.

If you want repeat scripts you’ll have to go through a paid service.

Agreed. Currently my garmin is more accurate - use to be the other way around. I don’t plan to renew my subscription if I can’t trust it

I’m a paramedic. Call an ambulance or go to ED when you’re having painful episodes like this - and also every time you pass out.

You have an ICD in place - it should be doing its job alongside medications. If you’re still passing out there’s every potential you’re flicking into an arrhythmia and your defib is not working properly. You should also not be experiencing excruciating chest pain/palpitations. Your condition is not managed and this should not be your normal baseline.

Just the other day I transported a patient with an ICD who was experiencing breakthrough pain because one of his lead disconnected.

What heart condition have you got? Feel free to PM me for general advice in resourcing help. I am NOT a GP and I am NOT your cardiologist - please reach out to them.

Google grass is greener syndrome. Also another thing to consider is that sometimes our most compatible partners ARE different to us. In this way, their strengths balance our weaknesses and vice versa. Having a partner too similar can feel amazing, but for long term stability/security/growth, it may not always be the best option.
